To schedule an annual safety inspection for a vessel, please complete the form below and send a request via email to audit@liscr.com, and include the following information:

1. The vessel name and official number.
2. The ports the vessel will be calling upon.
3. The expected date of arrival.
4. The agent’s contact information in that port.
5. Attach a copy of the current crew list.
